Overview

Laparoscopic sleeve gastrectomy, sometimes called gastric sleeve surgery, is a type of weight-loss (bariatric) surgery that helps severely obese patients lose weight by allowing them to feel satiated despite limited food intake,. During the procedure, about 80% of the stomach is removed. This surgery prompts hormonal changes that help with weight loss and relieve conditions that are linked to obesity, such as hypertension and heart disease. The achieved weight loss can greatly reduce the risk of developing obesity-related diseases and improve life quality.

Sleeve Gastrectomy: Reducing Risk of Weight-related Health Problems

The primary aim of this procedure is to help you shed that extra weight and reduce your risk of developing potentially life-threatening weight-related health issues. These include:

  • Hypertension
  • Heart disease

Who is Eligible

Sleeve gastrectomy is an option for people who have tried but failed to lose weight through proper diet and physical activity. You can have this procedure if:

  • You have a body mass index of 40 (37.5 for Asians) or higher (morbid obesity)
  • You have a body mass index of 35 to 39.9 (32.5 to 37.4 for Asians) with at least one weight-related health condition

Pre-Op Diet for Sleeve Gastrectomy

The effectiveness of the surgery will significantly depend on your pre-operative and post-operative diet. Your diet is crucial to the success of the procedure for three main reasons:

  1. It helps reduce fatty deposits around the liver, which enhances surgical access to the stomach. Hence, the diet makes the procedure easier and safer.
  2. It also lowers the risk of complications. Since obesity increases the risk of complications during and after surgery, weight loss can help in reducing that risk.
  3. It helps you start a long-term commitment to weight control.

Ideally, you should start your pre-operative diet for a sleeve gastrectomy three weeks before the procedure. For this diet, you must plan to:

  • Limit calories, especially carbohydrates. Certain sources of carbohydrates, like pasta and bread, are not necessarily bad, but reducing intake can help you lose weight.
  • Maximise your protein. Consume 60 grams of protein or more daily. Good protein sources include eggs, chicken, lean ground beef, and salmon.
  • Integrate healthy fats into your diet. Some fats are bad for your health, while others are not. Healthy fats are found in foods like nuts, fish, and olives. Consumption of other fats, like trans fats and the ones found in butter and oils, should be cut down.
  • Keep hydrated. Drink plenty of water, and avoid soft drinks and alcoholic beverages.

Some surgeons may prescribe the use of protein meal replacement shakes as the pre-operative diet.

As you are nearing your surgery, you will need to switch to an all-liquid diet. You must restrict your intake to broth, gelatin, water, and low-calorie sports drinks (avoiding sodas). Starting at midnight on the day of your sleeve gastrectomy, you will need to avoid food consumption entirely. Failure to follow these guidelines may jeopardise your eligibility for the procedure.

Advantages of this procedure over other types of bariatric surgery include:

  • Preserved stomach function because the stomach and outlet valve nerves are not changed
  • Less risk of developing vitamin and mineral deficiencies because the food still follows the normal digestive path
  • Weight loss averages of more than 50 percent of the excess body weight

Sleeve gastrectomy usually uses the laparoscopic approach (small incisions are made in your abdomen where a telescopic camera, instruments, and staple devices are inserted). The advantages of this approach include the following:

  • Reduced need for pain medication
  • Less risk of wound infection
  • Shorter hospital stay
  • Quicker recovery and return to normal activities

Your doctor will instruct you on what to avoid and when you can resume your daily activities. It is recommended that patients take three to four weeks off after surgery. They can resume more vigorous activities after four to six weeks.

The answer varies for every patient. The amount of weight you will lose depends on several factors. These include diet, physical activity, lifestyle, and your overall commitment to breaking bad habits.

Sleeve gastrectomy patients lose 60% of their excess weight on average. Excess weight is usually computed by subtracting your ideal weight from your current weight. Your excess weight will then be multiplied by 60% to get the expected weight loss. Finally, to calculate your new weight after surgery, you just have to subtract the expected weight loss from your current weight.
