Laparoscopic Liver Surgery

Dealing with liver conditions, whether benign growths or more serious tumours, can feel overwhelming. You may be managing ongoing health concerns, facing anxiety about surgery, or simply unsure about what to expect. While open surgery is usually the norm, minimally invasive techniques now allow many patients to go for liver operations without the need for big cuts in the body. Laparoscopic liver surgery offers the advantages of shorter recovery times and less discomfort.

What is Laparoscopic Liver Surgery?

Laparoscopic liver surgery is a minimally invasive procedure used to treat various liver conditions, including benign lesions and liver cancer. Unlike traditional open surgery, which requires a large incision, laparoscopic liver surgery uses small keyhole incisions and specialised instruments, including a laparoscope—a thin tube with a camera—to remove or treat abnormal tissue within the liver.

This approach reduces post-operative pain, recovery time and scar size, making it an attractive option for eligible patients.

What are the types of Laparoscopic Liver Surgery?

The type of laparoscopic liver surgery recommended depends on your condition. Common procedures include:

  • Laparoscopic Liver Resection (Hepatectomy): Removal of a segment, lobe, or section of liver tissue affected by cancer or benign tumours.
  • Laparoscopic Cyst Fenestration: Treatment for large or symptomatic liver cysts by draining or removing the cyst wall.
  • Laparoscopic Ablation (e.g., Radiofrequency Ablation): Destruction of cancer cells using heat, typically for small tumours.
  • Single-Port Laparoscopic Surgery (where suitable): A more advanced technique using a single incision, often around the navel.

When is Laparoscopic Liver Surgery recommended?

Laparoscopic liver surgery is commonly used to treat:

  • Benign liver lesions such as haemangiomas, focal nodular hyperplasia (FNH), and hepatic adenomas.
  • Malignant tumours, including hepatocellular carcinoma (HCC) and liver metastases from colorectal or other cancers.
  • Symptomatic liver cysts or abscesses that do not respond to medication.

Not every case is suitable for laparoscopy and open surgery may be preferred. Your doctor will have to consider your tumour size, its proximity to major blood vessels and any complications that may arise because of previous abdominal surgery.

Procedure for Laparoscopic Liver Surgery

Before the Procedure

Before surgery, you will undergo a series of evaluations, which may include:

  • Blood tests to assess liver function
  • Imaging studies like CT, MRI, or ultrasound
  • A consultation with an anaesthetist
  • Instructions for fasting before surgery and stopping certain medications

During the Procedure

On the day of surgery:

  • You will receive general anaesthesia.
  • Several small incisions (typically 0.5–1.5cm) will be made in the abdomen.
  • A laparoscope and surgical instruments are inserted through these incisions.
  • The surgeon will carefully isolate and remove the targeted liver tissue or lesion.

The procedure usually takes 2 to 4 hours, depending on complexity.

After the Procedure

Here’s what recovery is generally like after laparoscopic liver surgery:

  • You may stay in hospital for 2 to 5 days.
  • Pain is typically well managed with oral medication.
  • Most patients can resume light activity within 1–2 weeks, and return to work in 2–4 weeks, depending on their job.
  • You will receive detailed post-operative instructions, including dietary guidance and wound care.

Follow-up appointments will be scheduled to monitor your recovery and review pathology results if applicable.

Risk Factors for Liver Conditions

Laparoscopic livery surgery is generally safe but some underlying conditions may increase your risk of developing liver lesions or tumours:

  • Chronic Hepatitis B or C
  • Non-alcoholic fatty liver disease (NAFLD/NASH)
  • Cirrhosis
  • Excessive alcohol use
  • Family history of liver disease or liver cancer
  • Certain inherited conditions (e.g. haemochromatosis)

Cost of Laparoscopic Liver Surgery

The cost of laparoscopic liver surgery in Singapore varies based on several factors, including the complexity of the procedure and the type of liver lesion being treated. Quoting from the Ministry of Health Doctors Fee Benchmark, the doctors’ professional fees are as follows:

For laparoscopic liver resection (hepatectomy)

Surgeons’ fees typically range from $12,000 to $33,600 (excluding GST).

For laparoscopic cyst fenestration

Surgeons’ fees usually range from $9,300 to $13,700 (excluding GST).

For laparoscopic ablation (e.g., radiofrequency ablation)

Surgeons’ fees generally range from $2,700 to $4,700 (excluding GST).

These figures reflect only the surgical fees. The final hospital bill will also depend on:

  • Your choice of hospital
  • Length of hospital stay and recovery needs
  • Charges for anaesthesia, diagnostic tests, or other services
  • Your insurance plan’s co-payment, deductible coverage, and claims limits

All procedures are Medisave-claimable, which can help cover part of the cost. Additional support may be available through your integrated shield plan or private insurance. For a cost estimate tailored to your situation, please consult with your hospital’s financial counsellor or care team.

Laparoscopic Liver Surgery FAQs

As with any surgery, risks include some blood loss, infection, bile leakage, and injury to surrounding structures. However, complication rates are generally low in experienced hands.
